LifeWise Essentials 20 Plan in Arizona

For consumers who prefer lower premiums, the LifeWise Essentials 20 plan provides coverage for major services such as emergency care and diagnostic services, with up-front benefits for preventative care and immunizations. This plan offers deductibles of $2,500 or $5,000, with 20% co-payment for network care and 50% co-payment for non-network care.

LifeWise and Health Savings Accounts (HSA)

LifeWise offers two plans that can be used in conjunction with a Health Savings Account—LifeWise HSA-20 and LifeWise HSA. Each is a high-deductible, low-premium plan, with the HSA-20 including a deductible of $3,000 along with 20% co-insurance, and the HSA a deductible of $5,000 with no co-insurance and lower premiums.

Small and medium-sized businesses can opt for PPO and HSA plans that are tailored to the size of the group. These plans include optional dental coverage that provides coverage for preventative care as well as diagnostic and major services.
