• Autograph plans are flexible high-deductible plans with 70%, 80% or 100% of costs of network care provided after deductible. Deductibles range from $1,500 to $6,000 for individuals and $3,000 to $12,000 for families (higher deductibles apply for non-network providers).These plans may be used with a Health Savings Account.

• Health Savings Accounts are available in conjunction with a qualified high-deductible health plan. These accounts can be funded with pre-tax dollars, and withdrawals for qualified medical expenses (including deductibles) are tax-free.
