Health Net of Arizona offers HMO, PPO and HSA plans, each of which offer variable deductible, co-pay and co-insurance requirements, as well as dental, vision and
supplemental Medicare plans.
- HMO plans require that you choose a primary care physician to coordinate your healthcare and refer you to specialists. Choosing an HMO plan means all your medical care must be obtained from network providers. Plan options include a deductible-free plan and one with a $1,000 deductible with lower premiums, both with co-insurance requirements of 30%.
- PPO plans offer a higher degree of flexibility, as you do not need to choose a primary care physician, and can visit specialists without a referral. You can choose to visit non-network providers, but these visits are more expensive, and non-network visits have a higher deductible.
- Health Savings Accounts include a savings account that you can make tax-free contributions to, and then use the money to pay for healthcare. This account must be used in conjunction with a high-deductible PPO plan. These plans are a good option for people who want to make their own healthcare decisions and prefer a plan with a lower premium.
- Optional dental and vision plans are available for any individual or family enrolled in a medical plan. The dental plan has no network provider requirements, and no referral is required for specialist care. The vision plan offers reduced co-payment costs for visits to network providers.
