Founded in 1939, Blue Cross and Blue Shield of Arizona (BCBSAZ) is an independent non-profit health insurance company. The BCBSAZ headquarters are located in Phoenix, with additional offices in Tucson, Tempe and Flagstaff.

Blue Cross & Blue Shield Products
BCBSAZ offers a range of medical and supplemental plans for individuals, families and businesses. These include HMO, PPO and Indemnity plans, as well as HSAs. Each plan offers varying levels of co-payment and deductible requirements.
The BlueSelect HMO plan does not include a deductible, while for PPO and Indemnity plans, the deductible amount ranges between $250 and $5,000.
- For individuals and families, BCBSAZ offers standard HMO plans as well as PPO, Indemnity and HSAs (in conjunction with BluePreferred or BlueClassic high-deductible plans). In Maricopa and Pima counties, discount dental plans are also available.
- Businesses may choose from traditional HMO and PPO plans, HSAs (in conjunction with BluePreferred PPO plans), and supplemental indemnity-based dental plans. In addition, employers are able to choose from two different plan levels, with the option to choose between low-premium/high-deductible and high-premium/low-deductible plans.
- Supplemental Medicare plans for those over 65 provide benefits for those enrolled in both Medicare A and B. There is no waiting period for pre-existing health conditions with these supplemental plans.
